I've noticed that the rank of Lieutenant seems to be a default in fanfiction. I seriously think that most folks in fandom have no idea how rank works in the military.
I can see how Bones would get a promotion (even in the real military, advanced degrees let you climb the ranks faster), but Gaila? A Lieutenant directly out of the academy? No.
Unless someone has an advanced degree or other reason to start with a jump in rank, they'd start as an Ensign. Then, they go to Lieutenant Junior Grade. Then Lieutenant. Then Lieutenant Commander. Then Commander. THEN Captain.
That's for officers. Now, there are also Warrant Officers, who are often called "Chief." That would include transporter chiefs, and so on (like Chief O'Brien in TNG). And then there are crewmen, who are support staff but not officers. They'd be addressed as "Crewman." But in the very least, almost all of the folks who got field-promoted due to the emergency with Vulcan would be called "Ensign" at best, or still "Cadet" really, especially 3rd-year cadets. But yeah, "Ensign Gaila," if not "Cadet Gaila." And so on.
